Declaration of consent to the application for support services in accordance with Articles 9, 13 and 14 of the GDPR
Data Processing
We collect personal data from parents, children and young people in need of support in order to review their application for support and ask for their consent by signing.
For this purpose, the following categories of data, some of which are sensitive, are stored in our data registers in accordance with Art. 9. The data is collected directly from the data subjects, is not passed on to third parties and is stored for at least ten years in order to comply with legal requirements.
Personal Data
From children and parents: surname, first name, date of birth, gender, bank details, email address, family and financial background, support contribution.
Health Data
From children: diagnosis, description of illness, state of health.
Additional information
Applicant, use of funds, comments, board decision.
Your Rights
Information
You have the right to obtain information about the data we store about you.
Correction, Deletion, Restriction
You have the right to correction, deletion, restriction and data portability with regard to the data we store about you. Due to the statutory storage period of ten years, your data can only be deleted after this period has expired.
Objection
Even after signing this consent form, you have the right to object without affecting the lawfulness of the processing carried out on the basis of the consent until revocation.
